- This position is hybrid with Tuesday-Thursday being in-office.
- The office is located in Chicago, IL.
- The work schedule is Monday-Friday, 6:45 AM-3:30 PM with required on-call support.
Overview
Our client is seeking a Healthcare Nurse Case Manager to join their team. As a Healthcare Nurse Case Manager, you will be responsible for coordinating and managing the care of patients with complex medical conditions or disabilities. You will work closely with patients, healthcare providers, and insurance companies to ensure that patients receive the appropriate care and support to improve their health outcomes. The average caseload is 30+.
Responsibilities
- Assessing patients’ medical, psychological, and social needs.
- Developing individualized care plans in collaboration with healthcare providers and patients.
- Coordinating healthcare services, including medical treatments, therapies, and consultations.
- Monitoring patients’ progress and adjusting care plans as needed.
- Providing education and support to patients and their families.
- Advocating for patients’ needs and rights within the healthcare system.
- Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
- Evaluating the effectiveness of care plans and making recommendations for improvement.
- Documenting patient assessments, care plans, and interactions.
- Keeping abreast of healthcare regulations, policies, and best practices related to case management.
Required Skills & Experience
- Registered Nurse (RN), Nurse Practitioner, Licensed Physician Assistant (licensed in Illinois).
- 2 years of experience in occupational health nursing or related fields.
- Experience with telephonic case management for short term disability.
- Knowledge and understanding of the provision of case management for workers compensation, disability, limited duty, safety, industrial hygiene, healthcare plans, regulatory, medical requirements, and external healthcare resources.
- Ability to work in and contribute to a team environment.
- Basic computer and cell phone skills.
Preferred Skills & Experience
- BSN, CCM, COHN, MSN, MPH or Master’s in related fields.
- 3-4 years, depending on experience and work history, of occupational health nursing or related fields.
- Evaluation, assessment, solving, and negotiating skills.
